1. Definitions
- Practice: Katendrecht dental practice, located at Brede Hilledijk 696, 3072 NH Rotterdam.
- Patient: The person who uses the practice's services.
- Treatments/Services: All dental treatments and related services offered by the practice.
2. Appointments
- Make appointments: Appointments can be made by phone, email or in person at the practice.
- Cancelling appointments: If you want to cancel an appointment, this must be done at least 24 hours in advance. In case of late cancellation, the practice can charge a no-show rate.
- Arriving late: If you arrive late for your appointment, the treatment may be shortened or a new appointment must be made. This is to reduce waiting times for other patients.
3. Payments
- Rates: The rates for treatments are based on the standard rates set by the Dutch Health Authority (NZa).
- Payment term: Payments must be made within the payment period stated on the invoice. In case of late payment, the practice may charge statutory interest and collection costs.
- Insurances: It is the patient's responsibility to check which treatments are reimbursed by the health insurer and to take the necessary steps for claims.
4. Treatment plan
- Drafting a treatment plan: For certain treatments, a treatment plan can be drawn up, describing the nature and extent of the treatment, the costs and the expected duration.
- Treatment plan approval: Before starting treatment, the patient must agree to the treatment plan. This can be confirmed orally or in writing.
- Changes: Changes to the treatment plan can be made after consultation with the patient and with mutual consent.
5. Responsibility and Liability
- Patient responsibility: The patient is responsible for providing accurate and complete information about his or her health condition. This is essential for determining the right treatment plan.
- Liability practice: Dental Practice Katendrecht is not liable for damage resulting from incorrect or incomplete information provided by the patient. However, the practice is liable for direct damage caused by negligence or errors of practice during the execution of the treatment.
- Complaints: If you have a complaint about the service, we request that you report this to the practice as soon as possible. We strive to resolve complaints satisfactorily.
6. Privacy
- Processing of personal data: The practice processes patient personal data in accordance with applicable laws, as described in our Privacy statement.
- Confidentiality: All employees of Dental Practice Katendrecht have a duty of confidentiality with regard to patient data.
7. Changes to the General Terms and Conditions
- Adaptation: Tandartspraktijk Katendrecht reserves the right to change these terms and conditions. Changes will be published on our website and will be effective immediately.
- Entry into force: By continuing to use our services after changes to the terms and conditions, you agree to the new terms.
8. Applicable Law and Disputes
- Applicable law: These general terms and conditions and all agreements concluded between the patient and Katendrecht Dental Practice are exclusively governed by Dutch law.
- Disputes: Disputes will initially be submitted to the competent court in the place where the practice is located.
